PROCEDURE FOR ADMISSION, DISCHARGE AND TRANSFER
ADMISSION GUIDELINES:
Applicants for admission to Hudson Valley Senior Residence must meet admission standards outlined by the
New York State Department of Health and the Board of Directors of the Home.
Applicants shall not:
- Require continual medical/nursing care
- Suffer from serious and persistent mental disability
- Be likely to cause danger to him/herself or others
- Disrupt or impair the well-being, safety and orderly operations of the facility
- Be chronically bedfast, unable to self transfer or chronically require the physical assistance of another person to transfer
- Chronically require the assistance of another person to walk
- Have chronic unmanaged urinary or bowl incontinence
- Be dependent on medical equipment which is not self managed
- Engage in alcohol or drug use which results in aggressive or disruptive behavior
- Be under 18 years of age
Applicants must complete an application and present it to the Home.
Applicants must present themselves to the administrator and /or admission committee for a pre-admission interview.
The applicant's physician no longer than thirty (30) days prior to admission date must complete a medical evaluation (DSS 3122).
In case of an applicant from a hospital, psychiatric center, skilled nursing facility, or health related facility, the sending facility will supply the necessary medical information. The sending facility will also supply psychiatric information and an individual service plan where applicable.
Prior to admission, the Hudson Valley Senior Residence will document the applicant's social and personal needs and the facility's ability to meet such needs. This will be done by the administrator or case manager.
At the time of admission, the Hudson Valley Senior Residence will execute with and provide to the resident an admission agreement, which will be maintained in the resident's record. This record will also contain the information submitted at the time of the pre-admission interview and any subsequent information, medical or otherwise, that would assist staff in meeting that individual's needs.
DISCHARGE AND TRANSFER PROCEDURE
Any resident may discharge him/herself from the Hudson Valley Senior Residence by providing the administrator with a written thirty (30) day notice. The Hudson Valley Senior Residence may terminate the Admission Agreement upon a thirty (30) day notice to the resident.
In the event that any resident develops a medical condition requiring skilled medical services, the
Residence will make immediate arrangements for transfer of that resident to an appropriate medical facility.
The following transportation services can be utilized:
- Ambulance service (private)
- Car service as provided by the family or facility if arranged in advance and available
At the time of the transfer, a complete emergency transfer form will accompany the resident. This form will contain pertinent administrative and medical information concerning the resident's reason for transfer.
In the event that a resident exhibits symptoms of mental illness to a degree that he/she constitutes a danger to himself or others, the facility shall arrange for appropriate professional evaluation of the resident's condition, and if necessary assist in the transfer of the resident to a facility which will provide the appropriate level of care.
Upon the transfer of a resident to a health, mental health or other residential care facility, all pertinent records shall accompany the resident, except under emergency services which prohibit such transmittal, however the above mentioned emergency transfer form must always accompany a resident. In such emergency cases, pertinent information shall be telephoned immediately to the receiving facility and written material sent within 72 hours.
